Welcome to the 2008 Corner Bakery Cafe Breadhead Loyalty Survey.

We are committed to seeking your feedback to better understand our strengths and where we must improve so that Corner Bakery Cafe continues to be a great place to work. We have partnered with Service Management Group (SMG) to administer the survey and to assure your responses remain COMPLETLEY CONFIDENTIAL. Under NO CIRCUMSTANCES will any data linking individuals to their responses be given to anyone at Corner Bakery Cafe. You are encouraged to be completely honest.

You were provided with a paper or email invitation that contained your unique login number. Please enter the number in the space provided and then click the “Next” button.

Your survey should take approximately 15 minutes to complete. We strongly encourage you to set aside enough time to complete the survey without interruption. After 20 minutes of inactivity, your survey session will time out requiring you to start over at the beginning. This is done to assure your confidentiality and security.

At the bottom of each page, a “Progress Bar” will reveal how much of the survey you have completed. Your survey is complete when you reach the “Thank You” page. When you reach that page, simply click the X in the top right hand corner of the page to close the window.
